Being in the pharmaceutical industry I can tell you the following -
A medicines shelf life according to any Pharmacopoeia is dictated by the purity that it can maintain of the drug itself that is contained in the formulation (tablet, capsule, etc) over a period of time. Usually the innovator brand (in this case Zithromax) dictates the shelf life of its generic version after thorough stability tests. These tests are performed in accelerated conditions with increased humidity and temperature (every 15 days testing for the first 3 months then monthly for another 3 months, which is as long as accelerated studies last to translate results as being safe for 3 years) and real time at room temperature and reduced humidity which is tested until 6 months as every 3 months, then after a year intervals. The US Pharmacopoeia dictates that 95-105% of the claimed dosage must be contained or else the drug is deemed ineffective. Strictly speaking this is not true. The efficacy is merely reduced marginally over the period of 2-3 years in the case of this drug to where the drug breaks down to less than 95% of its content. So safely assuming 90% remains after 3 years, you would still ingest 900 mg (1 gm dosage), 450 mg (500 mg dosage) or 225 mg (250 mg dosage). This is how antibiotics and life saving drugs which do not become harmful are donated after expiry to poorer third world countries where the norms are less stringent and drugs are in shortage.
The drug is guaranteed a potency which lasts until the expiry date. More than likely, being only 2 months past the expiry date, it will be OK. But, you are taking a chance and if it was for a cure of a STD (not, say for strep), I would get a new tablet.
No, please discard the expired medicine. Taking expired antibiotics result in resistant bacteria colonies in your body and may be dangerous for you.
Is it safe to take azithromycin if it expired 2 years ago
Yes
6 months after expiry date
It depends on individual tablets and when it was produced. Check the label and it will show the expiry date
