Whatever colour they happen to be painted (one can not see the siren usually). I hope you are not confusing the siren (which makes the noise) with the flashing lights (which are blue).

Depends on patient condition. If there is an immediate threat to life or limb, then the lights and siren should be used. Otherwise, they shouldn't. They come with an inherent risk of distracting other drivers and possibly causing accidents, and in today's "I'm the most important person in the world" society a lot of people won't even pull over anyway. Oh, and everywhere has the same laws - if you're running either lights or siren, you run both. Never run a silent lights, because it's illegal.
