A head stone is at the head.
NO! The body is first ritually cleaned, blessed, wrapped in a plain white sheet, and the deceased is buried lying down on his back in a regular wood coffin, barefooted, in a regular grave (in a Jewish cemetery - one which has been sanctified), and with a headstone that is unveiled one year from the individual's death. Said headstone should be no bigger than the average of all the others in the cemetery so as not to raise the status of the deceased over any of the other souls who rest there. No flowers at the gravesite either. Visitors/mourners place a small stone on the headstone to indicate that they've paid their respects.

Vince Lombardi and Bill Walsh.
Anyone can set a headstone if allowed by the cemetary, so you will need to check with their rules. Some require liability insurance and others mandate you must use their people per their rules and regulations (perpetual care cemetaries usually do not allow anyone to set a stone/memorial unless they are insured and bonded). This is not a simple task, but at the same time, it's not hard either if you know what you're doing.
