Theres a chemical that tells your body when to eat, and when not eat. If that chemical goes out of control, the child or say, person, can become overweight. Even if they don't eat much, the chemical can tell the person's body to store the food as fat.

It is most unlikely for a 2year old to understand the concept of anorexia. Failure to thrive is a more likely cause of an underweight child. This needs to be investigated to rule out acute illness, metabolic disorders, parasites etc. The child should see a Doctor or Child Health Nurse for assessment and referral.
