The wiper arm should have a plastic plug that pops off using a little screwdriver. Once that is off there will be a nut you can get off with a socket. Just shimmy the arm back and forth until it lifts off after that point.

I don't see it listed in the aftermarket guides so most likely its a Chevy-only part and you have to go to the dealer. Do not simply measure it and buy the same size - it probably won't work because if there was any chance, the aftermarkets would list it and they don't.
(1) Pull the rear wiper arm cover back to access the wiper arm retaining nut and detach the nut and pull the wiper arm straight off the shaft. (2) Open the rear hatch glass and remove the rear wiper motor cover. (3) Disconnect the electrical connector from the motor. (4) Remove the motor mounting screw and remove the motor. Installation is the reverse of removal.

First Raise the rear wiper blade assembly out away from the back glass it will pull away from the body of the van. next there is a plastic clip located on the wiper shaft pop this clip off. After you have removed the clip slid the wiper assembly out of the slot. This should allow complete removal. Next step re-install the new wiper in reverse.
