It appears that your thermostat is stuck in the open position. Replace it and all should be fine. It would be a good time to flush and refill the cooling system if you haven't done so recently.

There could be many reasons. Not enough coolant or bad flow, faulty thermostat, low oil. If the coolant freezes then it will overheat. If your car continues to overheat then you should have the coolant flushed and thermostat replaced.
THe fluid will need flushed and changed.
Check the temp sender and make sure the wiring is connected. How long has it been since the thermostat was changed and the coolant was flushed? You might try those...or at least a thermostat change. The coolant level is too low, and/or there is not enough water mixed with the coolant.
You might have bad actuator that opens and closes airways.
change your thermostat.

Blown head gasket is a distinct possibility, unless you just flushed and changed the coolant. In that case it is trapped air escaping.
Low coolant? System air bound? Hoses collapsing under pressure? Water pump not circulating coolant?
