It probably means the date you were notified that the account was in default. I do not know if you are referring to a Federal or State bankruptcy form. I don't know if every District is the same, but in Indiana the Court prefers the debtor to list the last date on which the credit card was used by the debtor or an authorized user. So, in the "date incurred" line on Schedule F it might say "Last used May 2004." The reason the Courts here like this is because it shows the Court how long it has been since you used the credit card so the Court knows you haven't used it in at least the past 60 days. Sometimes I list the date the credit card was opened originally and the last date it was used if I have this information, but at the very least I always list the last date it was used.
