While most people have a bally button that is an innie, some people have an outie, which is a belly button what protrudes outward. The reason for outies is just simply extra scar tissue. They can also be caused by umbilical hernias.

If the umbilical cord is cut farther away from the baby it's an outie. If it's cut farther in, it's an innie.
Very shallow innie. It was an outie when he was a kid but sunk in a little as he got older. It sinks in when he gains weight but pokes out when he has abs. With enough pressure, it could pop back out to a full outie again.
It's to do with the pressure of the tissue beneath the bellybutton; if someone has a flattie, or a very slight outie, then the tissues behind may either push it into an outie (say for example they breath in, pushing their tummy, and therefore bellybutton out too), or suck it into an innie (say for example they breath out, causing a slight vacuum suction to pull the bellybutton into their tummy). This can be artificially reproduced as well, as all people with true outies can push their outies in, and many people with true innies can pull their bellybuttons out to become an outie
