Consider thanking her for being a wonderful mother to your children, for giving you your children, etc. Something centered around the children would be very appropriate if you're divorced. If you can't plan something to do for her with your children, a framed picture of the children could be a thoughtful gift. But focus the sentiment on her as a mother and not on her as your ex-wife.
