Though stated in the body of the original Constitution, Article II, Secotion 1, the 25th Amendment (XXV) further clarifies that the Vice President is always the direct successor to the President (in the event of his/her death, or other special circumstances elaborated on therein).
Additionally, the 20th Amendment (XX) provides that in the event of the President-elect's death (again, among other things) prior to being swort in, the Vice-President elect is to then serve as President in his place (from innauguration day until the end of the original term 4 years later).
The main body of the constitution specifes that the vice-president become president if the president dies. (The 25th amendment provides a process for temporarily transferring presidential power to the vice-president if the president is incapacitated and can not perform his duties, but is still alive and has not resigned.)
It is not an amendment but the original Constitution which states that the Vice President becomes President upon the death of the President.
Article II, Section 1, Clause 6 states:
"In Case of the Removal of the President from Office, or of his Death, Resignation, or Inability to discharge the Powers and Duties of the said Office, the Same shall devolve on the Vice President, and the Congress may by Law provide for the Case of Removal, Death, Resignation or Inability, both of the President and Vice President, declaring what Officer shall then act as President, and such Officer shall act accordingly, until the Disability be removed, or a President shall be elected."
The 25th Amendment modified this a little. Section 1 of that amendment states:
"In case of the removal of the President from office or of his death or resignation, the Vice President shall become President."
Prior to the 25th Amendment some people argued that the Vice President wasn't really the President when the previous President died, but just the "acting President" - but it was and is still generally accepted that the Vice President really did become President, and each Vice President who has had to step in when the President died has been sworn in as President (not "acting President"). The 25th Amendment didn't really change anything in that respect but it did make what had always been the case even more explicit.
This statement appears in the main original Constitution. Article 1, section 1, clause 6. and was not added as an amendment. Amendment 25 allows for the vice-president to temporarily take over if the President is disabled.
No approval is needed for the vice-president to take over if the President dies. This is part of the original Constitution (Article 2, section 1, not an amendment). Amendment 22 gives additional procedures for temporaily transferring presidential power to the VP.

If a President dies and the Vice President takes over, the new President would appoint a vice president, authorized by the 25th amendment.
The Twelfth Amendment
The 12th amendment has this effect. It provides for separate elections of the president and vice-president.
The twelfth amendment to the US Constitution was intended to prevent having a President and Vice President voted in who were at odds with each other. Formerly, the Vice President was the presidential candidate with the second most votes. The amendment made it so that the Vice President was voted for separately and required a majority of the votes.
