It is caused in Chronic Viral Hepatitis B. It is due to an abrupt 5 fold rise of an enzyme called ALT (alanine aminotransferase) from the upper limit of normal. It is believed to be due to body's response to virus infection and is mediated through white cells present in the blood. This condition usually develops during or after anti-viral treatment or sometimes spontaneously.
