The nurse's first responsibility is to ensure the safety of the patient. After that, the nurse needs to report his/error. Always follow facility policy, but more than likely, you will report the error first to the physician and second to the nurse manager/charge nurse. An incident report/documentation of the error will also need to be completed.

There are 5 safety considerations and responsibilities that a health care provider should consider:

  1. You must ensure that it is the correct patientyou are administering the drug to. You should double check the identity of the patient against all relevant documentation.
  2. You must ensure that it is the correct medication that you are administering, again to the correct patient.
  3. You must ensure that you are administering the correct dose of the correct medication to the correct patient.
  4. You should ensure that you are using the correct route of administration, whether it be an enteral or parenteral route; with the correct dose of the correct medication to the correct patient.
  5. Finally, you should ensure that you are administering the correct dose of the correct medication via the correct route to the correct patient at the correct time.
