Generally speaking it is essential for a commander of an army to keep a portion of his troops in reserve. One is to prolong and renew a battle by bringing reserve forces into play at the right time. Another vital reason is have a reserve ready in case of any unforeseen threats. In the US Civil War, many of the Union generals were cautious and kept a proper amount of troops in reserve. This became an issue with US President Lincoln. He often criticized his generals for keeping too many soldiers in reserve. To him, it allowed far too many battles to be lost as these troops could have been used in offensive actions. The problem in this was that Lincoln was not a trained military man, while most of his generals were West Point graduates.

How are people in a war able to survive for so long and make it back home safe if one shot can kill or seriously injure someone?

There is no one answer that can totally cover that question, but one of the biggest reasons is that the heaviest combat is on the front. Therefore, almost all commanders will hold the majority of their troops in reserve, leaving a relatively small portion of the troops to bear the brunt of the fighting. Another big reason is that due to recent advances in technology (armoured vehicles, body armour, medical treatment, etc.), a bullet today is far less likely than a bullet from 1812, or even 1914.
