Short term goals - this is your immediate and for the next year or 2 plans. i.e. if you are applying for a specific job then your short term plan could be to gain the position you are applying for and your longer term plan would be some kind of promotion within that company.
Long term goals tend to be for 5-10 years from now and you need to be able to provide evidence of how you plan on getting there, i.e. have you enrolled on any further education/vocational training courses. Have you, or are you attempting to gain further qualifications etc.
That's basically what it means in short so you need to be able to demonstrate:
a) what you have done to prepare yourself for the next year or so and how do you plan bettering yourself in the next 5 years or so.
What they are looking for is your ambition and that you are career minded as opposed to just going into work and doing the basics of the job.
Let's see I plan to finish high school, Go to college to become an ultrasound tech. Married and kids :) I want 6 kids
You can always say that you would like to consolidate your position in a role such as XXX and then take on extra responsibilities and move up the ladder.

Usually the question is worded a bit differently with the interviewer asking where you see yourself five years from now. What they really want to know is what your goals are, how much enthusiasm or ambition you have for your career, and possibly how much responsibility you're eventually willing to take on. Give an honest answer, but concentrate on the growth you expect to make or the goals you plan to reach.
Most people see themselves in management positions in 5 years. Other see themselves rising above the management level or see themselves in a different job.
5 years after getting an MBA? Hmm, probably getting promoted to the Managerial position at McDonalds.. ;)
