They grow from the matrix which is a flap of skin like tissue under the eponicium. Pressures above below push the nail out like "a tube of toothpaste". Grooves under the nail attach through pressure to the nail bed.

both of my children's nails grow downwards and they have a corresponding heart anomaly. They have a bicuspid aortic valve and narrowed aortic arch. Their cardiologist said that these anomalies often go hand in hand. They were born that way, and their toenails will always grow that way. You may want to look into it. Mine were lucky to have it discovered early while some people don't find out until there's a problem and they're in their 50's. Good luck and I hope that it simply is your shoe as this other person claims.
