If it has been 3 weeks, you should consult your doctor, there could be a secondary bacterial infection at the site of the injection or other reason for the pain that could and should be treated.
It doesn't ease the pain of the prick if that's what you want. But it does start to work and once you've taken the shot, and you feel pain it would be less than if you hadn't taken the pain killer, this is because it has already started working inside of you. It basically eases the pain after the shot.

I have the same exact pain in my left arm. I had an MRI and it shows a slight tear in the Rotator Cuff, but after 2 cortisone injections I'm still in pain. I don't have the answer, but I am going back to the Orthopedics later in the month. I will see what they say during that appointment. My pain seems worse at night, but never goes away.
