I'm assuming you are using a sand filter. First, is the pump and filter hooked up corretly? Pumping the water through the filter the wrong way will blow sand into your pool. Is the control valve in the correct position? If it's in between positions it might let some sand into the pool. Is the control valve cracked or broken? Self explanitory.

Filter Sand in the body of the pool usually means that one of the laterals, that's the part that lets the water through after it's passed through the sand, or another part in the filter is broken.

Could be several things. If your pool walls are sunk into ground instead of setting on the ground, or pool is at base of a slope in the yard, and sand is washing out from under pool floor, it might just be a spot where heavy rains let water flow under pool wall and floor eroding the sand under the liner.

Usually small pin-holes in the liner won't do much if anything to the sand, but if you get a larger hole in the liner, the water leaking out of the pool could also cause erosion of the sand under the liner.
