What vehicle is the blade on? If it is on a lawn mower, You did not properly attach the blade but got the washer and lock washer in the wrong order. If it is a windshield wiper, you may have forced the blade and attached something upside down or stuck it in wrong. It should have gone in smoothly and you forced it in wrong and put it in the wrong way.
