Crater Lake was formed when Mount Mazama collapsed during a cataclysmic volcanic eruption and formed a crater-like hole called a caldera.

In Native American legend sky god Named Skell and Llao ( god of the underworld) were battling. While they were The Mountain was destroyed and the remains of it fell and made what we know today as crater lake!

Depends on which Crater Lake you are asking about.

A Crater Lake can be caused by two different things, 1, a volcano, 2, a large impact crater such as a meteor strike.

A good example of a volcano lake would be Crater Lake in Oregon. While the impact craters forming lakes are not generally known as crater lakes, a good example of this is Lake Manicouagan in Quebec.
