You need to file an amended return if you omitted a W-2 form from your original return. Form 1040X is Amended U.S. Individual Income Tax Return. You should file your amended return as soon as you discover any omissions or corrections.
Be sure to note where to mail your Form 1040X in the Where To File page of Instructions for Form 1040X. Amended forms don't necessarily go to the same Internal Revenue Service Center where you filed your original return.

If you're a taxpayer who's filing a paper return, you attach the federal copy of each W-2 form to the income section of the return. There's a note to the left stating that on the tax return. If you're filing electronically, you don't send your W-2 forms. But you're expected to keep them in your possession along with all of your other tax records for that year. If you're an employer, you must mail Copy A of Form W-2 to the Social Security Administration, Data Operations Center, Wilkes-Barre, PA 18769-0001 by March 1, 2010. But if you're an employer, you also can file Copy A electronically by visiting www.socialsecurity.gov/employer and selecting 'Business Services Online'. Then it isn't due until March 31, 2010. If you're an employer, you must provide W-2 forms to all of your employees by February 1, 2010, either by mailing to their addresses on file with you, distributing them at the workplace, or by making them available online for your employees to download.
If you discover an error after your return has been filed, you may need to amend your return. The IRS may correct errors in math on a return and may accept returns with certain forms or schedules left out. In these instances, do not amend your return! However, do file an amended return if there is a change in your filing status, income, deductions, or credits. Also, if the Form 8938 (PDF), Statement of Specified Foreign Financial Assets, applies to you, see Filing Form 8938 after filing 2011 or 2012 annual returns in the Form 8938 Instructions.
