If it is after the launcher but before you actually get into a household, it could be a few things. First, if you have custom content, you need to ensure that there are no corrupt files in it. Easiest way to do this is to (temporarily) move ALL of the custom content from your game folder. Once this is done, try booting up your game. If it starts without issue, you have a corrupt file in your downloads and will have to sift through it to find out which one is borking your game. With custom mods, you should check to see if there are any updates from the creator of the mod. This could be an easy fix that could save you a lot of time. If the error still occurs, you have a different problem which is hard to assess without more details. Sometimes, it could be as simple as re-installing and you're good to go...other times, it is a driver (graphics card is the usual culprit) that needs to be updated.
If it is while you are actually playing a household...well, it could still be a corrupt download, but the chances are slightly lower. If it happens when you do something specific and EVERY TIME that you do that thing, corrupt custom content would most likely be the issue. If the thing that is being done is interacting with a custom object then you have an easy task. Find that item in your downloads and remove it. This should solve the problem. If it is random, try re-installing.

You cant add another expansion pack.. when you didn't update your game... all you have to do is reinstall which basically means to update. Then you should be able to install world adventures.
