What does it mean if you have a knot on your back right by your shoulder that only hurts if you touch it but when you touch it it hurts real bad what could this be?

you could have just pulled or strained a muscle somehow without even realizing it, i have done this myself because i have a pretty physical job, so i pretty much hurt myself the same way. it will go away eventually if you try to get some rest and not do too much activity that could pull on it.
