The heart rate increases because of your bodies cells need more oxygen during activity. Nerves in the heart make the pacemakers fire electrical impulses faster because they release epinephrine and neropinephrine and thus the heart rate increases.

Cardiac output is the volume of blood pumped by the ventricle per minute. It is equal to the stroke volume times the heart rate:

CO = SV x HR

cardiac output = stroke volume x heart rate

Stroke volume is the volume of blood pumped each time the ventricle contracts. Heart rate is the number of beats per minute.

So, cardiac output can be increased by increasing either the stroke volume or the heart rate, or both.

During exercise, the initial increase in cardiac output is due to an increase in both heart rate and stroke volume. When the level of exercise reaches 40% to 60% of the individual's capacity, stroke volume reaches a maximum. Further increases in cardiac output are largely the result of increases in heart rate.

Think of it like taking steps. Your overall speed is the cardiac output. Stroke volume is how big each step is. Heart rate is how fast you take each step. You will be going the slowest when you take small steps slowly. You will be going faster if you take large steps slowly, or small steps quickly. Finally, you will be going the fastest if you take large steps quickly.

Stroke volume is controlled by the heart. If more blood enters the heart, more will be pumped out. This is because the heart muscle cells will be stretched more, so they can contract more. If you want to know more, Google the Frank-Starling Law.

Heart rate is controlled by the brain. Sympathetic nerves from the brain release chemicals which increase heart rate. Parasympathetic nerves release chemical which decrease the heart rate. These chemical are called neurotransmitters. Sympathetic nerves release Adrenaline (epinephrine in the USA), and the parasympathetic nerves release Acetylecholine.
