Nothing happen's but the child will live with the tag of "father unknown" for the rest of their life. Paternity is presumed if the parents were married at the time of the child's birth. If the parents were not married, paternity can also be established by signing an acknowledgment of paternity or/and by genetic testing (DNA).
Unless you are married the mother is the one who gets to name her child. The father cannot avoid responsibility for his child by not signing the birth certificate. He can be compelled by a court order to submit to a DNA test to determine paternity. Once paternity is established the court will enter a child support order and a visitation order if the father requests one.
You have larger problems than that with the relationship. Find another name that both of you can live with. Often what seems like a perfect name to one person has connotations or memories associated with it to the other. There are many good names out there, so stop stonewalling and find that alternative!
Single fathers have no rights whether he signs or not, but he's still obligated to pay support.

He has no rights or legally anything to do with you.
you will be denied the child
no
Yes, if he is the one who fathered the child. Father's do not typically sign the birth certificate though, although he can be named on it.
yes =)
The mother can sign her name, not the father's name.
I know that in the state of pennsylvania, the father must be present to sign the babies birth certificate in order for the baby to have the fathers last name.
Putting a father's name on the birth certificate does not make him legally the father if you are not married to him. You can put his name on the birth certificate, but realize that it has no legal impact. In order to have paternity established, it has to be done with a court order.
Only the biological father can sign his name, if that is what you mean. If the child is adopted the adoptive father can change it and sign his. If the mother sign his name or another man's name, it is fraud. The mother must always be listed on the birth certificate.
No, he is the only one who can sign it and if he chooses not to that is it.
Only if you intend to acknowledge that you're her father.
When I had my daughter, her father was not able to be there at the time, so I was the only one to sign the birth certifiate as a parent. The father's name can be added by the father at a later date if that is what either of you wants to do. No one but the biological parents can sign a birth certificate.
yes
