The President appoints a new VP with Congress's approval. If the President and VP were to both die or leave office suddenly, then the Speaker of the House of Representatives becomes president and a new VP is appointed. The order of presidential succession goes:

President

VP

Speaker of the House

President Pro-tempore of the Senate

Cabinet (by order of when the positions were established)

In case a sitting US President dies in office, the Vice-President becomes President. No one automatically becomes VP. Historically, the office of VP has remained vacant for the remainder of the term, and has only been filled is that President is reelected. After Kennedy was killed and Johnson became President, there was no VP until Johnson's next term began.

If the President and Vice-President both die, the Speaker of the House becomes President and the President Pro-Tempore of the Senate would become Vice President.

The Line of Succession then continues through the Cabinet Officers in the order in which the Cabinets were created; beginning with the Secretary of State and ending with the Secretary of Homeland Security.

You will never see all 19 of these people together. At any event which all Government Officials must attend, one will be absent; having been place in a secure area.
