First, if you've got a safety-sensitive job like truck driver, pilot or whatever you CANNOT do your job with traces of MJ in your system even if you've got a prescription for it. (I know trucking so I'll discuss it here: Every trucking company is required to have a medical review officer. It can be the secretary and in a lot of real small trucking lines it is, but there has to be one person designated as the MRO. That person has to look at every driver's fitness for duty. If a driver is on a drug that can affect his driving, the MRO must put the driver out of service until he's clean again. If you go to the dentist and get Tylenol 3--a perfectly legal drug--you cannot drive a commercial motor vehicle until you've finished your prescription and the codeine's completely out of your system. If the cops catch you on the road with Tylenol 3 in your system they will suspend your CDL for three months if it's the first offense. Now...if they do that for a relatively benign drug like Tylenol 3, what do you think they'd do if you came up hot for WEED?)

Assuming you are in a place where medical marijuana is permitted, an appendix removal is not a valid reason for it. After having it removed, there is no reason why you would need the marijuana, since it doesn't cause any extreme pain and has no need of the various effects often quoted as beneficial of marijuana.
