Peeta was cut on his upper leg by Cato, the District 2 tribute. According to Katniss, the cut went straight to the bone. Because of not treating the wound properly, infection set in and blood poisoning resulted. He also received multiple tracker jacker stings on his upper body.

Later in the book, Peeta's leg is mauled/ripped badly by one of the wolf muttations. The wound is so bad Katniss must create a tourniquet to staunch the flow of blood that, if allowed to continue, would have killed Peeta.

Peeta's leg was hurt so badly, at the following Victor's interviews Katniss discovers he has a prosthetic leg.

In the first book,

He is said to have a slight limp, bruises on his face, five tracker jacker stings, a deep laceration on his thigh caused by another tribute named Cato, various scrapes and a bad infection which causes him to have a high fever and no appetite for food. It is also hinted that he may be nauseous.

Near the end he nearly dies of blood loss from a gaping wound in his calf.

He then acquires a prosthetic leg as a replacement.

Second book,

He's not as beat up in this one...

But, he is badly shocked by an electric force field in the arena.

He forms scabs and mental disorientation from poisonous mist. Later he is nearly impaled by monkey fangs, but is saved by a morphling from district 4.

He is said to have violent nightmares.

Third book,

He is mentally "hi-jacked" by president Snow's people and tries to kill katniss.

In the Capitol, he is wearing handcuffs and his wrists are scarred from pushing his wrists against them.

He's pretty messed up even after the book ends.

After the incident with the tracer jackers, Peeta tells Katniss to run, and Cato stabs his left leg he's so mad. In the book Katniss recalls hearing a " long high pitched scream that never breaks for a breath." I am currently in the middle of reading it for a third time, and the second time I read it I realized it might have been Peeta.

Cato, Katniss, & Peeta are chased into the cornucopia when a mutt cuts a gash in Peetas leg. Cato takes hold of Peeta and Katniss shots and arrow at Cato's hand, causing him to fall to his slow, painful death by mutts. Once Peeta and Katniss are the only ones left, Claudius Tempelsmith announces that they looked over the rule book and rules are rules. So out of an act of love/rebellion, Katniss pulls out night-lock and threaten for both of them to commit suicide. Just as the Berry's get in there mouths, Claudius Tempelsmith says "Stop! Stop! Ladies and gentlemen, i am pleased to present the victors of the 74th Hunger Games Katniss Everdeen and Peeta Mellark! I give you-- the tributes of District Twelve!". Katniss and Peeta then spit out the night lock and are taken to the capitol. Before Peeta and Katniss are reunited on stage in front of the entire Capitol to be broadcast live to all of Panem, Haymitch tells Katniss that the Capitol isn't pleased with her and Peeta's attempt at double suicide, but as it was her idea, she'd be the one to pay the consequences. He explains to her that she must tell everyone that it was her love for Peeta that made her want to eat the berries in an effort to get the Capitol to believe that it wasn't actually an act of rebellion like they think, though Katniss is conflicted and does not know the real reason why she did it. On their way back home to District Twelve, Katniss hints to Peeta that the way she acted toward Peeta (in love) was "all for the games." Peeta becomes upset and hurt and escapes to his room on the train until they arrive home and must play the "star-crossed lovers" angle once again. Katniss misses Peeta, even in that short time, because she realizes she really does care for him but she's not sure in what way. She says she doesn't "want to lose the boy with the bread."
