It depends on how violent the batterer is and if they have no fear of the law then yes, a restraining order is basically worthless. If the batterer threatens you by phone; email or stalks you (appears everywhere you are) then report this to the police. If the batterer has been arrest before there will be a case report on him and you can call the police for help. If you are deathly afraid of this person then there are Women/Men's Abuse Centers and you can call your Mental Health to get the phone number. They will guide you through the process and keep you in a 'Safe House' and even take you to court if need be to press charges against your batterer.

The husband and wife should both go to individual therapy (not couple's counseling- that can be dangerous for someone in an abusive relationship) to help them deal with their separate issues. They can also each go to a different batterer's intervention course if they are willing.
The best thing is to go to a "Batterer's Intervention Class" if Anger Management doesn't work. The books should be for the victim. There is a book called "Why does he do that?" by Lundy Bancroft that I HIGHLY recommend. If he wants to change, then support him in taking the classes. Ask him questions.
