Epitaph poems seldom had a title for they were written not on paper for books or for publication but were meant to be chiselled or inscribed on plaques over burial places and tombs. Anyway, due to their poetical quality or strangeness, many of them found their way into books with appropriate titles added. There have been the strangest of poems written as epitaphs. Here is one which is written by Samuel Johnson in his boyhood when he walked over a duckling and killed it.
Here lies good master duck
Whom Samuel Johnson trod on.
Traditionally it is the subject of the poem, the theme, the meaning or perhaps the most important line in the poem. In actuality, it can pretty much be whatever you want - it is your poem after all :)

Only you can come up with a good name! Titles come from the story (or poem), not from some anonymous person on the internet! You're getting this backwards I'm afraid -- you need to write the book first and think of the title last.
Titles are also not as important as some writers think - your title might change many times between your first write and your final draft!
Think about your poetry - what is going on and who are the people involved? What is something special about the book that might make a good title? Don't sweat it - just write something down and get on with the story!

Well if it was a poem about school than i would say maybe somthing like SCHOOL DAYS or maybe STUCK IN SCHOOL.

A good name for the book would be "Forever" - but it's already been taken by Judy Blume, who wrote a book by that title on exactly the topic you described.
