This is usually nothing to worry about even though it can be extremely distressing to the male in question.
Angiokeratoma of the Scrotum is in fact bleeding from the Fordyce Spots.

The Fordyce spots (dark red 2 mm papules) on the scrotum and base of the penis can sometimes bleed. It is usually limited to just one spot though. If it is regularly happening then referral to a dermatologist would be appropriate. Localised laser treatment can prevent further bleeding from the spot.

Angiokeratoma are spots on the skin, usually red, that are a normal part of aging and the human condition. The scrotum is a common place to find them.
