The outcome for extreme Narcissism is not good since narcissistic personalities are among the hardest to treat. Their disorder flows from a belief that they can do no wrong. That conviction makes therapy especially difficult because they are unwilling to acknowledge their problem. Therefore, individuals with narcissistic personality disorder rarely seek out treatment and if they do, at the urging of family or partner, they do not stick with it since it's a long term commitment with the goal being the reshaping of their personality.
Since the narcissistic has a rigid, and skewed pattern of thinking and behavior that leads to problems with relationships, education, careers and social interaction. They usually blame others for their problems. They assume no adult responsibilities and expect indulgence from others. Because they are unable to modify their own behavior they are doomed to repeat their mistakes, failures and the behaviors that lead to them over and over again.
Complications of untreated NPD can include:
- Substance abuse

- Depression
- Inability to persevere in a job or career
- Problems with relationship
- Suicidal thoughts
- Difficulties at school and at work
- A magnification of their behaviors as they age
