For the Reverse problem, you could have low transmission fluid, or a bad transmission. For starting in drive, just about every modern car will not start when the transmission is in gear. this is a typical problem with berettas, for some reason the bands in the transmission will not hold out after several miles are on the car, have the bands tightened or replaced, and you wont have any more problems
Check your relay under the dash. My 1991 JCB did the opposite and that is what the problem was.
I just read another persons witing on another site about this. He was having the same problem I am with his Pathfinder. We all wont go in reverse. His turned out to be the solenoids.

It is most likely a internal problem. If the seals in the reverse clutch drum are dammaged it will slip hot cause the fluid is thinner when it gets hotter.
There is probably a problem with your selector i had the same problem on my 5 speed boxs the other day change the clutch and the reverse light switch but for got to put the washer on the switch so it whent in to far and caused the reverse link not to go in
