I've been searching for the same thing and have come to the conclusion that there is no section for this. What you're supposed to do (I believe) is find the source of the questionnaire and cite that. For example the Big Five Personality Inventory was originally included in a journal article. You need to find the original article and cite that, rather than the inventory itself.
