What is the difference between Responsible and accountable?

I have been researching the proper usage for responsible and accountable. Although both appear interchangeable, I'm finding that responsible works better to describe a pre-event and accountable is best used post- event. Also similar to the difference between less and fewer; responsible tends to be used in a general sense where accountable is more measurable.
