The lower control arm bushing would be located at the inner pivot point where the lower control arm is mounted to the frame/crossmember. Depending on your vehicle it could have one or two bushings. The outer pivot point would be your lower ball joint.

I believe it has to do with the force exerted on the pitching arm. Throwing all those fast pitches puts strain on the arm and bones, and slowly makes the arm grow longer.
If you mean the lower suspension arm (wishbone) ball joint, it is fixed into the arm. This means you need to replace the whole lower arm.
