I would say The Bible. Consider that the bible has been banned in various countries from ancient times until now. I believe it is currently banned in 52 countries. Historically it has been banned by many Communist and Muslim countries and even at times by The Roman Catholic church.
One of the most banned books in America is "The Catcher in the Rye" by J.D. Salinger. It has faced numerous challenges and bans in schools and libraries due to its language, sexual content, and themes of rebellion and teenage angst. Despite this, the book remains a classic in American literature.
While the bible does show up on lists of banned/challenged books, it isn't even close to the most banned of all time. I'm not sure there is one specific book, as the things conservatives freak out about change with the times.

Before even that though many European countries regulated the printing of books that disagreed with the current rulers or the state religion.
