Most states require guardianship petitions to be filed by an attorney licensed to practice in the state where the action is is filed. In states that allow "self-help" the person(s) wishing to obtain guardianship of a minor child file a petition in the circuit or superior court in the city or county where the child resides. The petitioner(s) are required to pay all court costs and accompanying fees. In cases where a minor child/children have been removed from parental custody by the court a GAL is appointed to represent them until the court appoints a legal guardian. When this applies any acceptable family member or interested party may file a request to be appointed guardian and will not need legal counsel nor be required to pay legal fees.

Should a pregnant teen be able to keep the baby even if the parent say no?

The parent is the parent and guardian of the teen but have no legal right to decide over the teen's baby. Teens have the same feelings towards their children adults do and have the same legal rights. So yes, of course. Forcing parents and children apart is wrong and can cause irreparable trauma.

What does a baby need to fly within the US?

A parent or legal guardian. A notorized medical power of attorney if not the parent or legal guardian. Copy of the birth certificate along with Mom's picture ID or copy , and a notarized letter of permission. Plenty of formula , food ,diapers, and askyour pediatrician for something if the baby has any tendency to motion sickness. Try to bottle or nurse the baby during take-off and landings to help offset the pressure in the ears. The swallowing helps. If your baby is fussy, ask the flight attendant if there is someplace you can go to get away from other passengers. Make sure Baby has some ID on their person. In an emergency, you could get separated. And keep a current picture with you, and one in your checked luggage as well.
