Pernicious anemia can be best diagnosed with a blood smear where red blood cells that should be round and concave, appear oval and are larger with thin cell membranes. Due to the dysfunction of these red blood cells, they are fragile and incapable of properly carrying oxygen through the body. This deficiency is rare and usually only seen with strict vegetarians, or people that have undergone stomach surgeries where a substance secreted by the mucosa in the stomach, called intrinsic factor, is produced. Intrinsic factor is needed in order for B12 to be absorbed in the small intestine.
