It's not a real coin or a minting error. It's a novelty item called a magician's coin. They sell regularly on eBay for a few dollars.
Magician's coins are made by cutting two genuine coins in half, then swapping and rejoining the sides. The result is one "coin" with 2 heads and another with 2 tails.
