I'm having the exact same problem with a '91 I just bought. According to the dealer, the previous owner had the problem for years with no other transmission issues. I have this problem starting at around 40 mph, and can't take the car over 60 at all.

Can 'Volcanic Rocket' be used to retrieve a 'Tri-Blaze Accelerator'?

Yes, it can. There is a difference between a "Blaze Accelerator" and a "Blaze Accelerator card". The first refers to a card called, and only called Blaze Accelerator, while the second means any card with Blaze Accelerator in the name. Therefore Tri-Blaze Accelerator is a 'Blaze Accelerator card' and can be searched out by Volcanic Rocket. Compare it to the various Dark Magician support cards. Because they work on "Dark Magician" and not a "Dark Magician card", they only work with a regular Dark Magician, not things like Skilled Dark Magician or Dark Magician Girl (unless specified).

How do you shift 18 speed gear?

If you're already familiar with unsynchronised transmissions, the shift pattern is basically a nine speed, with the option of being able to split gears, via the lever on the side of the transmission. When pushed forward, it puts you in the high side of the gear, and puts you on the low side of the gear when pulled to the rear. To shift between the two ranges, you simply let off the accelerator, pause, then press on the accelerator again. If you do it too quickly, it won't catch the gear. If you're not familiar with shifting these unsychronised gearboxes, you're not going to learn it simply by reading about it. You need to be trained on shifting nine and ten speed transmissions first, then wait until you've got it figured out before you get into the 13 and 18 speeds.

Our 2004 Avalon has a very jerky shifting action. When we bought it the RPMs were running high and that was fixed but the shifting is not smooth.?

is the car cold when you observe this? -> common problem, especially between 2nd and 3rd gear. Another thing i have observed is that if you accelerate with a lead foot or with a really light touch, they both produce a fairly smooth shifting. However, when you drive moderately, the shifting can be noticeable..and sometimes feel like being jerked.. running synthetic transmission like mobil 1 may helped a bit..
