I would first check the master cylinder, and slave cylinder. Not knowing anything about the auto, most use hydraulics to release and engage the clutch. A clutch disc will tend to slip when in higher gears when it starts showing signings of wear. Most car companies use dot 3 brake fluid in their clutch systems.

No. Clutch is what disengages the motor from the transmission. Shift would be changing gears. You should depress the clutch to shift from one gear to another.

try to make sure the clutch pedal is all the way down before you change gear. in no way shape or form should you have to floor the clutch everytime you shift gears, however, if you are still grinding gears while flooring the clutch, and if there isn't that little bit of give on the pedal i'd suggest adjusting or replacing the clutch.

Well first of all, if your talking about a manual shift, the clutch is. more or less part of the transmission. Sounds to me like your clutch pad / disk is worn out or you burned it up. Which can happen if you slip the clutch all the time at stop lights / signs. In that case you should probably replace the clutch plate and disk replaced, and have your flywheel resurfaced.
