Loctite PL Premium is a contractors adhesive that is 3 times as strong as mastic and ideal for this kind of application
Liquid nails would work well. ANS 2 The use of Liquid Nails on ceramic tile is time wasting and very expensive. The best adhesive for this is thinset mortar laid with a castellated trowel.
I would use PL300 or a similar general adhesive.

Depends what you want to stick onto it. Not many glues stick effectively to plaster.
Thinset mortar. There's a special one for porcelain tile; any thinset will work for regular ceramic tile.
It's best to call a glass specialist. The adhesive for new windshields can be very tricky to apply.
A good strong epoxy, Speedgrip, J B Weld or Araldite.

You should not adhere tile on top of tile. It is much better to rip it up and install on a secure surface and make sure it is smooth level. If you find yourself in a situation where you can not/should not remove the tile (e.g. if there is asbestos tile underneath or if there is a radiant heat systems under and you don't want to risk breaking/puncturing it), then the alternative is to pour 1/2" (or more) of self leveling cement and then install on top of that.
Best glue I've found for that is 'Sta-Put' spray adhesive, sold at stores that sell plastic laminates.
Whatever you do, don't try to repair it yourself unless you have a repair kit handy. It will just rip open again later. With a repair kit, it will provide you with adhesive, as well as help you match the color to the recliner if it has opened up too much.
