US - on private waterways/lakes. Some states do not require PFDs at all. On USCG patrolled waters - it depends on the size of the boat, there is a foot length or displacement, over which passengers are not required to wear a PFD.
A whistle or bell, AND a horn audible for at least 1 mile.
U.S. Coast Guard-approved visual distress signals.ALSO: An audible warning device - flotation equipment for the maximum rated capacity of your boat - navigation lights.
Admiralty means your vessel is operating in international waters.
alcohol/ blood test
To have a sample of your blood or breath tested for alcohol.
The answer depends upon whether the boat is under US or Canadian regulations, or something else, whether it is a motorboat, and how big it is, among other things. Not all boats are required to have a B1 fire extinguisher in the US or Canada.
