A transmission that causes the RPMs to increase without changing gears could be in need of fluid. If that does not fix the problem, it may be the car needs a new transmission. For safety reasons, it is imperative the car is taken to a trained mechanic to be diagnosed properly.
The most common cause for a transmission not to change gears, is low transmission fluid. Check the transmission fluid and fill to the full line.

If this is an automatic transmission, more than likely the transmission is on it's way to the junkyard. Usually failing transmissions lose reverse first, then the higher forward gears - last will be the lowest forward gears.

If the gears are slipping there can be a multitude of problems. A leak somewhere either in the housing or in the transmission lines could let air into the transmission which would cause it to not work properly likewise if the transmission is very old/high miles the gears are eventually worn down by metal filings in the transmission fluid you would need to change out the fluid to alleviate this problem. But, if there is just too much damage done your only recourse would be to swap it all out costing thousands of dollars in parts alone.
In an automatic transmission this usually means the trany is low on fluid or the trany has internal problems.
