The US President is sworn in at 12:00 noon local time on Inauguration Day, which is January 20 of the year following his election.

In the event of vacancy of the office (death, incapacity), the Vice President is sworn in as soon as practicable.

In modern times, the President Elect becomes President when he or she takes the Oath of Office on Inauguration Day, January 20 of the year following the election.

Section 1 of the 20th Amendment to the U.S. Constitution says,

"The terms of the President and Vice President shall end at noon on the 20th day of January, and the terms of Senators and Representatives at noon on the 3d day of January, of the years in which such terms would have ended if this article had not been ratified; and the terms of their successors shall then begin."

It is interesting to note that Abraham Lincoln (and all presidents 1793 - 1933) had to wait until March 4th to be elected. The Confederate President, Jefferson Davis, had already been sworn in (February 22, 1862) before Lincoln took office. The outgoing president, James Buchanan, the "Lame Duck" President, didn't want to get involved in the dispute between the states and Lincoln could not do anything because he was not yet President. On April 12th, Ft Sumter was attacked, a little more than a month after he took office.
