Initially have one extra stitch in the row. When you join, knit the last stitch together (knitting 2 together, in effect) with the first one.

I use four needles up to the point where I start the instep and divide for the heel, then I change to three, one for the instep and two for the heel, but only because I find it easier to follow my pattern for the decreases once I finish the heel.
