Vague question, isn't clear if this is about a problem, or if this is about how a chain works.
Basically what the chain does is to transfer force from the pedals to the rear(drive) wheel, thereby making the bicycle move. If you have external gears, then a gear change means that the gear has to move from one sprocket to another. If you have internal gears then the chain stays, and cogwheel inside the hub moves.
if you don't keep your chain and gears on your bike greased it will rust and come off.
A chain is a chain, it hasn't got any gears in it. But if you run it across a pair of sprockets you can create a gearing ratio.
Unfortunately, you can't change gears on bikes in HeartGold.
To change gears on the bike, all you have to do (while riding it) is press the B button. You don't have to hold it in, either. ~Lucario
A derailleur is a part on a bycicle that changes the gear by pushing the chain either up or down the sprockets.

If it has external gears you can't. They have to have the chain moving tp be able to shift. At best you can lift the rear wheel of the floor/ground, spin the crank, and change the gears. Remember to avoid cross chaining.
Depends on why it's jumping. It might jump because chain and sprockets are too worn, it might jump because chain tension is too low, or if its a bike with external gears it might jump because the gears aren't adjusted properly. Don't really know what you're tinking of with "mesh together"... On a bike with external gears the gears will not mesh. There's the chain acting as a go-between to transfer the energy. On an internally geared bike you will have cogwheels that are interlocking and driving each other.
