I don't believe there is any sample letter. I tell my customers to write a simple letter stating the facts. If it is simpler for them, I tell them to put in numerical order what happened before they start writing so they won't forget and try and add it in later confusing who is reading the letter. Such as--1. It started with medical decision-(whatever it might have been)or laid off as company downsized or death in family. 2. Had several procedures, or hospital visit/s, incuring large medical bills/deductibles. 3. Went on disability/lost job, had loss of income -- etc. 4 started going behind, marital troubles, decided to go bankrupt, attorney took longer than expected --etc. Then write the letter--do not make it too long as an underwriter will only want to read so much. Make it no longer than 1 page, type it as then there won't a problem reading the handwriting and spell check can be used. If handwritten, no longer than 2 pages, try to print so it will be readable. You want to make sure that whoever is reading it knows that it was a problem, that there will be a little sympathy and they understand that it was beyond your control.
