If it has an automatic transmission, it is added at the dipstick near the rear of the block on the driver side. if it has a manual transmission there is no dipstick.... climb under the van and add it with a pump.

According to the 2005 Ford E-350 Owner Guide : ( Motorcraft MERCON automatic transmission fluid is used as the power steering fluid )
The transmission housing for a 1991 E350 is bolted to the rear of the engine. The entire transmission case is considered the housing.
