Country code +45, dialed as 00 45 from many places, is Denmark.

Country code +5, dialed as 004 5 from many places, is an incomplete code somewhere in the Americas. You need one or two more digits to specify the country. For example, +55 is Brazil.

To avoid confusion, it is best to write an international telephone number in correct international format, beginning with the plus symbol and the correct telephone country code (e.g., +45 or +55), omitting any dialing prefix.

For example, what you might dial as 00 44, I might dial as 011 44 or 010 44 or 0011 44 or several other possible combinations. Thus, it is least confusing to quote an international telephone number beginning with the plus symbol and the actual telephone country code.

Telephone country code +44 is the United Kingdom, but there are no valid numbers beginning with +44 44.

That is not a valid telephone code in the UK. If the number you are dialling begins with 0041, it is in Switzerland, not England.
